In this report I will be explaining how the application of relevant principles and values will enable professionals to provide holistic support for individuals who use social services, review the benefit to individuals and professional staff, of taking a holistic approach to planning support and also analyse reasons for working with professionals from more than one agency when planning support for individuals.
Case studies
Kamala is an older lady, who lives alone. Her partner passed away and her family visit when they can, but this is not very often. Kamala is having difficulty with her memory, she forgets everyday things like whether she put the cooker on. Kamala also has been buying cat food, when she does not have one. She has diabetes and uses her medication, but her daughter is worried she will forget to and become ill. She had a career come and visit her, but she was unprofessional and kamala thinks she has been stealing her money. The career bought the wrong type of food and was quite patronising to kamala. Kamala is an independent woman, she always has been, so despite her needs she is adamant about staying at home.
Tom was born on 2 May, 1975 and he has Down’s syndrome. He currently living in Burwell with his mother, Jennie, a cat called Tig and a dog who is deaf. Tome has a part- time job in a café where he helps prepare the food. He also serves at the tables and works on the till. He loves to cook and sees himself as the next Jamie Oliver! He feels the job at the café has given him a lot of catering experience. Tom attends the local further education (FE) college one day a week and is studying on a retail course. Tom loves music, his favourite band is Westlife. He dream of singing on TV, hopefully with Westlife! On his days off, he enjoys going into Newmarket to watch a film or have a drink at the local with his friend. Unfortunately tom’s mom is seriously unwell, so can no longer support him as much as she did previously. Tom is hoping to move out soon and have a place of his own, he has always wanted his independence but now he has less choice about the matter. Previously tom moved and shared with his brother, Edward, for a short time. Nut he found that Edward was very messy and lived mainly on junk food. Tom felt that he had to take control and make the place habitable. He would like to stay close to his mother so that he can pop in to see her whenever he likes. Tom’s mom is supportive of him moving out but as she is unwell she is finding it hard to support him to do so. Tom does not want support at home as he feels moving out is important.
Principles and values describe the kind of attitude towards care you would appreciate if you were being cared for yourself. Creating a positive care environment requires health and social care workers to adopt principles and values which become a way of being and working.
The principles of care describe the way health, social care and early year’s providers should behave towards individuals in their care. A care worker should treat an individual in the way that they would like to be treated and they should work in the best interests of the individual.
Values are the beliefs about what is important to you as an individual, and what you belief about what is morally right and wrong. Values are usually learned from parents/ careers and tend to change throughout your life.
Principles and values include recognising and acknowledging the following points;
Empowering of individuals
Promotion of choice/ Recognition of preferences
Respect for religious beliefs, moral beliefs and values and culture
Anti-discriminatory practice
Promotion of rights

What does it mean/ how can it be applied to kamala and Tom and how it meets their holistic needs?
Empowerment of individual means giving enough information to enable them to make informed decisions and make choices about their life. Empowering kamala will enable her to take responsibilities for her own live by making informed decisions. In kamala case study it is important that the care professionals apply empowerment to the individual and the care value base, this means that professionals should give kamala and Tom enough information so that they will be able to make their own decisions and choices about what they would like to do. This will create a positive care environment. To apply empowerment to kamala and Tom, the care professionals would have to promote choice, this could be done by asking kamala and Tom what they would want to eat.
Promotion of choice/ recognition of preferences
What does it mean and how can it be applied to kamala and Tom and how it meets their holistic needs?
Promotion of choice means giving individuals the freedom to make certain decisions about their care and treatment given to them.
This enables professionals to provide kamala’s holistic support by looking at kamala’s emotional needs and what makes her feel more comfortable and willing when it comes to making decision about her treatment. If her emotion is considered it will make her respected and comfortable. This includes, the right to food, water, shelter and clothing and the right to be treated as an individual and to be respected. By giving choice to Tom about the food he eats, this will help meet his physical needs. This enables healthcare professionals to deliver holistic support through providing kamala and Tom with these fundamental requirements. Physically this will provide respect as kamala and Tom are being treated well and their choices are being provided. Intellectually allowing kamala and Tom freedom of speech. Kamala and Tom are being provided with dignity when they are clothed for and spiritually kamala is being respected regardless of her spiritual beliefs.
Respect for religious beliefs, moral beliefs and values and culture
What does it mean and how can it be applied to kamala and Tom and how it meets their holistic needs?
This will enable professionals to provide holistic support as kamala’s beliefs and values will be respected by all the professionals taking care of her, therefore applying to all aspects of holistic support. The individual’s personal moral belief’s religious beliefs, values and culture all have emotional, intellectual, physical, social beliefs and spiritual beliefs.
Anti-discriminatory practice
What does it mean?
Anti-discriminatory practice means treating a person or group unfairly because of particular characteristics, such as age, gender, skin colour, religious belief.
Anti-discriminatory practice can be apply in kamala and Tom and meet their needs by not stereotype kamala and Tom. From kamala case study above, she cannot speak English. The healthcare professionals must not stereotype her because she cannot speak English. A translator should be organised for kamala. professionals can promote anti discriminatory practice in many ways. This can be in ways such as making sure Tom is included in all activities and events happening in school. Even though Tom has a down-syndrome, it is important to ensure he is not isolated from any activities. Tom should not be discriminated due to his disabilities. If Tom becomes friendly with other friends and can socialise with them, he will feel happy and confident during his time in school.

Confidentiality means keeping or being kept secret or private. Confidentiality is the protection of personal information.
This will enable professionals to provide holist support as the professionals will ensure that all documents concerning Kamala and Tom’s are kept confidential. Document such as, medical information, their addresses, their care plan and information concerning their family members. By doing this, it will meet kamala and tom’s emotional needs.
Promotion of rights
What does it mean? How can it be applied to meet kamala and Tom needs?
Promotion of right means making sure that individuals are given their human rights and the ability to perform these right.
This enables professionals to provide holistic support by making sure that kamala and tom is given their human rights and not infringed. Kamala and tom must be informed of their own and other individual’s rights in order to make sure no rights are being are being abused. This will make kamala and tom feel confrontable, safe and happy.

In a medical setting holistic refers to addressing the whole person, including their physical, mental and emotional health. Providing holistic support to kamala and tom can have a major effect on their recovery. It is important for professionals to create a care plan that looks holistically to kamala and tom’s physical, emotional, intellectual, social and spiritual needs. The care plan will ensure that every need of kamala and Tom are met.
Care plan should involve kamala and Tom as much as possible. This will empower kamala and Tom to make their own decisions. Kamala and Tom will feel empowered and in control of their own care when they are involved in their own care plan. With holistic support professionals can produce an accurate care plan for kamala and Tom. If kamala and Tom suffering from emotional problems such as, kamala being only and losing her husband, tom being emotional about things happening in his home, it is the social worker responsibility to address these problems. Kamala may have certain cultural and religious needs as well. health care professionals may allocate her a care worker that she will feel comfortable during her time at home since she wants to stay at home as well as someone who will understand her beliefs. Ensuring kamala’s physical, emotional, intellectual social and spiritual needs means she will be empowered and valued as an individual. Professionals who work with kamala and Tom will also benefit from person centred approach to their care. By ensuring all their needs are being met, they will be free from stress. With holistic approach, staffs will be able to get on well with kamala and tom.
Working in collaboration is essential if individuals are to be offered the range of support they require in a timely manner. Multi-agency working is about providing a seamless response to individuals with multiple and complex needs. This could be as part of a multidisciplinary team or on an ad hoc basis. You will need to be clear about your role and responsibilities, and understand the different structures and governance of colleagues from other sectors, including the private and voluntary sectors, micro and direct employers, service user led organisations and brokers assisting with support planning. Working across these boundaries is critical to planning and providing appropriate support. Effective multi-agency working can mean that the individual who are receiving care can get a better service and a better outcome from service providers. Teams make up the building blocks of health care and every team—from the executive to the coal face—is composed of different professionals, ideally possessing a variety of skills necessary to produce safe and effective care. We are constantly reminded of the value of diversity within teams, but the reality is that working together from a variety of perspectives is sometimes difficult to achieve.
For kamala professionals who will be working with her include; a nurse who will provide hands-on care to kamala, observing and monitoring kamala’s condition and communicate with her GP. The dietician will assess kamala’s nutritional needs and develop a food plan for kamala. The Pharmacist will play a role by preparing medications for her by reviewing and interpreting physician orders. The GP will focus on the health of the whole person combining physical, psychological and social aspects of care.
Professionals who will be working with Tom includes; A personal tutor at college, who will assist, evaluate, motivate, develop and integrate Tom’s learning. A key worker, who will ensure that all the Tom’s records are adequately set up and recording is taking place. A support worker, who will be providing emotional support by talking to Tom and listening to his problems and concerns and drawing up a care plan for Tom. a nurse, who will advocate for the best interests for Tom and to maintain Tom’s dignity throughout treatment and care. This may include making suggestions in the treatment plan of Tom, in collaboration with other health professionals.
Both kamala and Tom will need a social worker will be responsible for helping kamala and Tom, their families to cope with problems they’re facing to improve kamala and Tom’s lives.
When health and social care practitioner’s works together, they are likely to meet the needs of kamala and Tom and be able to spot any problems and issues which ensures they are being cared for in a holistic manner. Multi-agency working together reduces the number of medical errors and increases patient safety. Multi-agency working also reduces issues that lead to burnout. No longer is one person responsible for the patient’s health, an entire team of health workers comes together to coordinate a patient’s well-being. Multi-agency working helps break down hierarchy and centralized power of health organizations, giving more leverage to health workers. Each care providers have their own expertise that they specialise in, this means a range of knowledge will be brought to the table to improve the care they are offer to both kamala and Tom. Also, with a variety of carers involved in the care plan for kamala and Tom, opinions will be share which could result in new ideas being developed. Gaining a range of views during issues that relate to kamala and Tom, it will be easier to find a solution from different health and social care professionals rather than only one professional.
The features of effective partnership working are to ensure there is good communication between all parties that ensure a high standard of care is delivered to the individual involved always. It ensures each party is covering all area’s and that all needs are met and that all parties have a good knowledge of who is delivering what to the individual and the contacts they have if other issues may arise and the access they must have to these. In a multi-agency working equipment and facilities can be shared which avoids duplication of resources. However, it may be challenging at time due to many different ideas and conflict on a topic which they some may not agree with which means it will be difficult for them to come with a final solution. The teams must work together and communicate effectively to avoid problems for kamala and Tom. It is important for kamala and Tom to be aware of the power these teams have and that they don’t misuse their power. If kamala and Tom disagrees with the suggesting in their care plan, they have the right to let the professionals know about it.
This tells us that kamala and Tom have the right to what they want and to take responsibilities for their own live by making informed decisions.
In conclusion, multi-agency working convey many benefits to both the patients and the health professionals working on the team. These include improved health outcomes and enhanced satisfaction for clients, and the more efficient use of resources and enhanced job satisfaction for team members. To ensure optimum functioning of the team and effective patient outcomes, the roles of the multidisciplinary team members in care planning and delivery must be clearly negotiated and defined. This requires, respect and trust between team members, the best use of the skill mix within the team, agreed clinical governance structures, agreed systems and protocols for communication and interaction between team members.
